The selected measurement tools for the study included the Social Appearance Anxiety Scale (SAAS), the Social Media Disorder Scale (SMDS), and the UCLA Loneliness Scale. Primary care facilities in China, since 2009, have been instrumental in the strategic integration of chronic disease management (CDM) services into the essential public health care they provide. We sought to calculate the percentage of Chinese patients with chronic conditions who considered CDM services conveniently accessible at nearby primary care clinics in mainland China, and to examine its association with the EQ visual analog scale (EQ-VAS) score and the utility score of the five-level EQ-5D version (EQ-5D-5L). From June 20th, 2022, to August 31st, 2022, a nationwide cross-sectional survey of 5525 patients suffering from chronic illnesses was carried out in 32 provincial-level administrative divisions. The sample comprised 481% (n = 2659) females, with a median age of 550 years. The median EQ-VAS score, 730, was associated with an EQ-5D-5L utility index of 0.942.
